New Patient Form

For your first visit please fill out our New Patient form (need this form), be sure to include any allergies, medical history, or medications.

Insurance

If you have insurance, bring your insurance information for us to put into your file. We direct bill to most insurance companies. If you have your insurance information book, you can look under your dental place to see your limits and percentage of coverage as well.

Questions about your insurance? Give us a call, we would be happy to walk you through it. We can send a predetermination to your insurance to get a more accurate estimate of what percentage of your visit will be covered.

If you are concerned about the cost of the visit, please give us a call or email us and we can break down the estimated cost of your appointment for you.

Patient Examination

A full new patient exam with cleaning is approximately 1.5 hours. We do a full medical history check, take X-rays, records and have an in-depth look at your oral health. We will do an oral cancer screen, gum health check, assess what needs to be done, and will offer potential treatment for the future.

The hygienist will clean your teeth, polish them and administer fluoride.

Dentist Examination

We will walk you through our findings once you meet with your dentist. The dentist you meet will be your dentist for life. We do our best to keep your checkups with the dentist you saw at your first visit. If you have any preferences in the dentist, please let us know prior to your appointment.

After the Appointment

Your hygienist will recommend when your next visit will be based on your oral health and book your next appointment for you, usually 4-6 months down the road. This appointment can be changed, but it acts as a reminder for you and us to keep you on schedule.

If you have any treatment needed, the hygienist or assistant can book appointments accordingly.

If a referral is needed, that will be sent out accordingly.

If you require any major treatment, we will submit a predetermination to your insurance on your behalf. Generally, it will take 4-6 weeks to hear back about your coverage.

At the end of your visit, we will submit your bill to your insurance or let you know your total. If we have an estimate of what the insurance isn’t covering we will take the remaining balance then. If you have two forms of insurance, we will submit the second one as well.

On your way out you will receive a goodie bag of oral health supplies.

If you have any questions prior to your visit please reach out via phone, text, or email, we would love to assist you.

Kids

The dentist is not a scary place. Dentistry has changed since we were kids and Aspen Trails Dental wants to show children that the dentist can be a fun place to be.

We want to ensure children have a happy dentist visit. This visit is to help children feel comfortable at the dentist.

We offer the following services for children's visits:
  • Services for children aged 2 and under (every 6 months until they are comfortable)

  • Introduction to the office

  • Ride in chair

  • Introduction to tools and Mr. Thirsty (suction)

  • Able to play with Mr. Thirsty and look at a mirror

  • If the child feels comfortable we may even try a cleaning, X Rays, or dental checkup

Dental Education

Dental education and teaching proper oral health care for parents and kids are key. A happy dentist visit starts with the first few times your child visits our office. You can bring your child in as soon as you feel comfortable or as soon as their first tooth comes in! At this visit, we will show them the office, play with the tools, show them "Mr. Thirsty" (suction), and get them comfortable in the chair. We will put on a show, give them a ride on the chair and count their teeth if they allow us.

A happy visit is exactly that. We let them know that the dentist is a fun place. If they start to get worried or uncomfortable, we will stop there and get them a prize. We recommend happy visits every 6 months to keep up the momentum.

If you are bringing your children in for their first visits, the same protocol for covid screening applies for each person that will be present at the appointment. We ask that one parent joins their child for appointments, but understand that if it’s a child’s first visit, both parents may want to be present.
